Supervision contracts for each student are developed to define the roles and responsibilities of the faculty supervisor, site supervisor, and student during practicum and internship.

The following documents serve as the supervision contracts between the students and their respective practicum/internship course instructor, site supervisor, or faculty supervisor who provides weekly on campus individual supervision during Practicum:

a.  Course syllabi state the expectations and responsibilities of the students enrolled in the Practicum and Internship courses as well as the roles and responsibilities of the group supervisors.

b.  On Site Supervision Agreement Form explains the roles and responsibilities of the site supervisor and the agreed upon requirements in supervision between the student and the site supervisor. More detailed information on the roles and responsibilities of site supervisors is discussed on pages17-20 of the Field Site Supervisor Manual.

c.  In addition to the one hour weekly supervision session provided on site, the students enrolled in Practicum attend a one hour weekly supervision session on campus during Practicum. On Campus Individual Supervision Agreement Form explains the roles and responsibilities of the on campus individual supervisor and student as well as the agreed upon requirements in supervision.
